No, the household does not have to take out accident insurance for you. This is already included in the purchase price of a service cheque. 

Self-employed domestic helpers and cleaning companies
If you are self-employed as a domestic helper (with the appropriate business licence) or have a cleaning company, the household does not have to take out insurance for you. You are responsible for this yourself. If self-employment is confirmed, no accident insurance is required on the part of the household.
